Probably the face, that's what your looking at most often compared to the rest of the body.

In nightmares I could control my dreams so yeah! Here I come!

Would you rather be a sexual predator that feeds upon kids and the elderly or a serial killer that just kills anyone on sight? And note that neither might be insane.

Preferbly I'd be a killer in that instance... But you kind of attached killer with "anyone on sight"... so yea. :/

Broken limb. At least with that I'll still be capable to function, mostly, rather than be stuck in bed for 2 years.

It was how my Dad removed a loose baby tooth of mine one time.
I can't imagine it being nearly as painful as eye removal.

Jigsaw, for very slightly increased chances of survival, if he gives you a fighting chance to survive. (unless he just wants you dead; in that case it's a failed gamble.)

Trapped in a room. You never said it didn't have entertainment. Plus I get horribly motion sick. Spinning in a dark void likely wouldn't help my case.

Would you rather be 1) trapped in a world where your loved ones all die horrifically, and after they've all died, the world resets and you have to live it again for eternity
or
be 2) trapped in a world where your loved ones watch you die horrifically, and after you've died, the world resets and all parties involved have to live it again for eternity

In both scenarios you live your life until the point of the loved ones/your horrific death. So in theory you get to spend time with them before the horrific death(s)
