As far as I'm aware, the /char command has to be edited retroactively after it's been sent the first time.
That is, you have to send a message first, and then edit it to change the [USER] tag. In virtually any situation where malicious use of this would cause actual harm, I think (hope) that someone would see it before the person manages to change it.
That said, it is still a problem that we should probably address at some point.
