He isn't, as shown by the ridiculous amounts of food he eats. But his various physical ailments prevent him from gaining weight.

PERSONALITY
➳ Obnoxious, is the first word many use to describe Roman Hemlock. And they're not wrong -- he is very loud, stubborn, and cocky, though he would just label himself as confident. Roman is unapologetically and irrevocably himself, and he doesn't make any effort to hide any parts of his personality to anyone. In his opinion, he thinks everyone should be completely real and honest with themselves, like he is.
Roman is a very talented individual. With a musical and artistic mind, what Roman lacks in physical prowess he makes up for in his singing and painting abilities, both of which have won the school many of awards and have earned him somewhat of a reputation in the town. He is a showboat and loves to perform -- unfortunately, this means he never shuts up, although he has the voice of the angels so this isn't really much of a downside.
Despite his confidence, Roman is actually very cautious when it comes to physical activity. He has the most severe form of haemophilia and could literally bleed to death due to a nosebleed, so he steers clear of any and all sports and doesn't even dance that much in the musicals. He also never goes outside. Roman is very comfortable with his sexuality and, despite being bisexual, steers more towards the stereotypical sassy, snarky, fabulous gay friend. Though, due to his charismatic nature, he is not a sidekick, but the leader.
But in spite of his confidence and his talents, Roman Hemlock is hiding some pretty deep insecurities, packed so tightly away that even he isn't aware of them wholly. He constantly feels as though his body has been specifically designed to fuck him over, and thus, he clings to his physical beauty as if it is his only redeeming quality. Roman comes off as shallow and materialistic because of this, but he's actually the kindest and most altruistic soul in the world. He constantly tries to help other people, often at the expense of himself.
